BOARD OF HEALTH
Meeting Minutes
January 8, 2007
Memorial Town Hall

MEMBERS PRESENT:        Martha Waite, Mark Johnson, Mary Ricker

The Board of Health Meeting was called to order at 5:00 PM

MEETING MINUTES:
October 23, 2006:  M. Johnson motioned to approve.  M. Waite seconded.  Approved.
November 27, 2006:  M. Johnson motioned to approve.  M. Waite seconded.  Approved.

SEPTIC PLAN REVIEWS:

539 Quinapoxet Street (map 104/lot 10):  David E. Ross Associates for Jeffrey Bellrose.  The applicant is submitting septic plans for a repair.
Recommendation:
The Board recommends the use of an effluent filter.  If an effluent filter is to be used, the installer must supply the homeowner with care and maintenance instructions.
Conditions:
1.      Need to have three of the lot lines surveyed by an R.L.S. and staked prior to start of construction.
Decommission the existing septic tank and overflow cesspool in accordance with Title 5.

M. Johnson motioned to approve with recommendation #1, and Conditions #1 – 2.  M. Ricker seconded.  All in favor.

Lot 1 Sterling Road (12/5):  Jay Finlay of Finlay Engineering & Ron Thunberg of HUB Survey Assoc., Inc. for Theodore Zawalick Jr, new construction.  The applicant is re-submitting plans for approval.  This was tabled at the 9/18/06, 10/23/06 & 11/27/06 meetings.  J. Finlay explained that Title 5 recommends designing the system parallel to the contours to minimize the fill required for breakout.  A slope easement is required since they are over the lot line.  Part of the Zoning requirement is that no part can be set back more than that.  Although there is a total of 12 acres, they can’t change the lines and still meet the zoning requirements.  To maintain the lot width requirement; it’s an odd way to calculate the lot width, but is required per zoning in the town of Holden.

Recommendation:
1.      The Board recommends the use of an effluent filter.  If an effluent filter is to be used, the installer must supply the homeowner with use and maintenance instructions.

A slope easement shall be recorded prior to the start of construction.

OTHER BUSINESS:
Sweets & Java:  Sean Navin & Michael Karrigin discussed the on-going renovations to the former Lilly’s Bakery.  They have moved a wall to allow for 8–10 seats and public bathrooms, created a smaller kitchen with a three bay sink.  This is on both town water and sewers.  Perhaps an outdoor area at a later time.  They also run Café Dolce on Shrewsbury Street.  They plan to bring in prepared pastries from Boston and order locally.  Will be using mostly disposable utensils.  A pre-inspection is set for this Friday.

786 Princeton St:  J. Crouse to send registered letter to homeowner ordering repair of a failing system, and copy Jim Morin.

120 Moscow Rd:  J Crouse will send a registered letter to the property owner requesting completion of this filing.  Central Mass Septic to receive copy as well as separate letter informing him that his license will be suspended if this work is not completed.

Solid Waste Task Force:  J. Crouse to send letter to committee stating that the BOH supports the current recycling efforts of the Town and the schools should be required to participate in the efforts.

Sand Certification Forms:  J. Crouse asked the Board to consider including these forms in all septic filings.  BOH agreed to incorporate these forms as a way to track the sand being used in system installations.

Residential Kitchen:  Applicants are Kelli & Carrie Brewer, 174 Union St.  J. Crouse will review this application and conduct a pre-inspection.

CFM course for churches:  J. Crouse asked M. Ricker if she could research if any CFM courses are being offered that would allow one rep from each of the area churches to attend.

Merged Food Code:  J. Crouse distributed a copy of the Merged Code to the Board and informed them that she would be distributing these to all of the food establishments, at no charge, during her inspections.

BT Region II Public Health Coalition for Emergency Preparedness:  J. Crouse updated the Board on materials distributed by the coalition and the agenda for tomorrows meeting with reps from Police, Fire and the Coalition, as well as an Medical Reserves Corp. Spokesperson, Karen Card.  She will update the Board at the next meeting.

Outdoor Wood Fired Boilers:  J. Crouse updated the Board on the current bans approved by local Boards of Health.  M. Johnson asked that all correspondence regarding the boilers be sent to the Board members as well.  Further discussions will continue at the next meeting.
